[LV EV 4] Hepatectomy for HCC located at caudate lobe

Lecture : The caudate lobe is the posterior part of the liver that embraces the IVC. The lobe lies between major vascular structures such as middle and left hepatic vein, left portal vein and IVC. The caudate lobe can be divided into paracaval portion, caudate process and Spigelian lobe. Due to anatomic location, it is difficult to do isolated whole caudate lobectomy. Therefore, caudate lobe used to be resected together with left or right liver. But if the remnant liver function is not enough, we have to consider isolated caudate lobectomy. It is difficult to define the border of right side of caudate lobe. If isolated caudate lobectomy is done completely, we can find the right, middle and left hepatic veins superiorly and IVC inferiorly and hilar plate inferiorly after resection. Recently, laparoscopic caudate hepatectomy has been reported. Laparoscopic resection of the Spigelian Process is very good option. Another advance in liver surgery is ICG fluorescence imaging. This technique is very useful to find tumor especially in the caudate lobe
