Introduction : Introduction : Situs inversus Totalis is a rare congenital anomaly characterised by a reversal of the usual handedness of the visceral topography . It is accompanied by anomalies in various systems of the body . The variation in the sidedness of the gall bladder results in the difference in the presentation , diagnostic difficulty and the need for modifications in the operative technique .

Methods : Here we present a video presentation of laparoscopic cholecystectomy in an acute cholecystitis patient with situs inversus totalis

Results : Technique : There was a slight modification in the technique from conventional Lap cholecystectomy . Port placement was a mirror image of that done for conventional laparoscopic cholecystectomy . The surgeon and the camera person was standing in the right side of the patient , first assistant on the left side . Major port of dissection was done through the epigastric port with the left hand and the right hand was used to retract the hartmans pouch . The first assistant retracted the fundus of gall bladder laterally from the left side of the patient .the clip was applied through the umbilical port with the surgeons right hand and the hartmans pouch was retracted with the surgeons left hand . Camera was changed to epigastric port at at the time of clip application .Specimen was retrieved through the epigastric port .

Conclusions : Conclusion : Laparoscopic cholecystectomy is feasible and can be done by an experienced laparoscopic surgeon in a patient with situs inversus totalis with a slight modification in technique .
